WAIVER OF LIABILITY AND RELEASE AGREEMENT
 
I hereby assume any and all risk on my behalf of myself and those on the premises (of personal injury, sickness or death, as well as property damages and expenses of any nature whatsoever incurred by myself and my child or children) in usage of 3rd floor at 354 B St. I specifically release, hold harmless and forever discharge Victory Outreach, all government entities (any property or employees in association therewith), all religious entities, from any liability due to injuries or damages suffered as a result (direct or indirect of my neglect, willful, reckless, or intentional acts while at Victory Outreach. I have read and know that my signature on the bottom of this form is my agreement to release, hold harmless and forever discharge any liability on the part of Victory Outreach, directors, officers, employees, volunteers, governmental and religious entities, and agents, from all liability or claims for my own neglect, intentional willful, or reckless acts in participation resulting in personal injury, sickness or death in the usage of this facility.

I agree to make sure room in use is cleaned before leaving. This includes picking up and taking out trash. Also, vacuuming floor, taking all belongings with me, making sure room is locked and room is only in use for the time agreed. I also will be supplying my own instruments during the time of occupying this facility.
